By agreeing to participate in the Advanced Breast Cancer Survey program, I agree to use this online platform only for its intended purpose as specified in the Terms of Use below.
By agreeing to participate, I am attesting that I am a qualified health care professional in good standing.
I agree to participate on the platform to create, revise and edit the content required for the development of the final document.
I understand that my personal information and other data will be gathered as part of the program and handled according to the Privacy Policy.
I am also aware that the aggregate data from discussions and other information related to this program will be shared in a key learning slide deck. For this reason, I will ensure that it is acceptable to use any information I share for this purpose.
PRIVACY POLICY
When you register or create an account with us, you may need to complete certain fields and use a user name and password which provides us with personal information. We will use your personal information for the purposes of the completion of the advanced Breast Cancer survey. Your personal information will be shared with third-party companies collaborating on the program. Also, personal information may be disclosed to a third party if we are required to do so because of an applicable law, court order, or governmental regulation. If you wish to request access to, or correction of, your personal information; or to contact us regarding our collection or use of your personal information, please contact us via the support page.
The purpose of this platform is not to discuss, disclose, collect or store patient personal health information, and we ask that you refrain from sharing any patient identifying information. However, in the case of an adverse event or product issue, reporting will be required and any related personal information (generally defined as information that could permit or contribute to the identification of a specific person such as yourself or a patient), such as name, location, and/or health-related information, must be collected for reporting purposes and submitted to regulatory authorities. This information will only be collected and used by employees or authorized third parties for these reporting purposes. Adverse event information may need to be reported to health authorities in and outside of Canada or to foreign affiliates. Should the collection and use involve a transfer of the information outside of your country of residence, we contractually protect the processing of your personal information by third parties; however, any transfer is subject to foreign legal requirements, including requirements in other countries to disclose personal information to government authorities.
